Responsibilities:
Provide Consultation, recommendations and interventions to clients in their home using evidence based best practice.
Assess psychosocial needs of clients and their families in response to illness/changing care needs and their related ability to adjust and/or cope.
Counsel clients and their family members concerning identified problems.
Counsel clients and their family members concerning long range planning.
Provide short-term therapy to clients and family members.
Problem solve in collaboration with client and care team to create opportunities for more independence.
Education and Experience:
Master of Social Work Degree from a recognized university.
In good standing with the Ontario College of Social workers and Social Service Workers.
Minimum 2 years of Clinical experience.
Valid driver's license with access to a vehicle.
